Student Complaint Form

Student Complaints allow students to express and resolve concerns, dissatisfaction, or perceived injustice the student may have with a college employee (i.e. faculty, staff, or administrator). All complaints are considered informal until they are filed in writing using the Student Complaint Form. Once submitted, the form will be reviewed by the Dean of Student Services. Appropriate staff will be in touch with you. Note: This form is for student use only. Employees with complaints should follow procedures in the Policy and Procedure Manual. This form is not used in cases of Title IX and Sexual Misconduct.
